1. The wheel cup came off twice. Both times was when I powered off the grinder and lifted it from the floor waiting for the cup to stop spinning. Somehow that lets the lock nut become loose and come off with the wheel. I stopped lifting the grinder off the ground when I turn it off until the wheel stops and I have not had any more accidents.
